In DoubleClick for Publishers (DFP) Small Business, how do I create a report to show revenue by URL and/or utm_campaign?

With AdSense and Google Analytics you can see revenue by page level and filter by different utm parameters. We want to move to DFP but need to be able to see page level and utm level reporting. I have been trying for a week and I'm stumped.
